Diabetes Management Program

The Empire Plan is introducing a  Diabetes Management Program to help individuals living with diabetes or high blood sugar. Enrollees who recently received treatment for diabetes or a diabetes-related problem may receive a letter inviting them to participate in the program. Participation is voluntary, free of charge, confidential and does not affect coverage in any way.

Selected enrollees will receive a letter announcing the program, which is called Managing for Tomorrow. The introductory letter will be followed by a confidential survey about health status. The survey can be completed via a toll-free phone line, a secure web site or by filling out and returning a paper questionnaire.

After completing the survey, each enrollee will receive personalized educational material based on responses to the survey. Dependent children may also participate and will receive age-appropriate material.

Registered nurses will help program participants by answering questions, explaining treatment, locating community resources, and coordinating with doctors as needed.

The Diabetes Program objectives are to improve the overall health and quality of life of participants with diabetes, and to reduce the cost of care for catastrophic diabetes.
